In the current SeaMonkey 2.16, [Tools > Image Manager > Manage Image
Permissions] launches the Data Manager, which does not have a "Remove
All Sites" for image permissions.  Instead, you must step through each
domain, one at a time, to check ALL permission, not just image
permissions.  If you find an image permission, you must then check the
"Use Default" checkbox to undo a prior image permission setting.  All
this is clearly not user-oriented, which is why I submitted my prior
reply in this thread.

The Permissions Menu in PrefBar gives you the old Image Manager along
with the old Cookies and Popups Managers.
