If you can find one of those 8-bit ISA interface-equipped I/O cards and have
a DOS CD-ROM driver, you're good to go with a CD-ROM drive on an XT-class
machine.

If you look in the 1988 issues of the now defunct magazine called Personal
Computing, you might see the advertisement for that nearly $2,800.00 Vendex
HeadStart PC.  It was your typical 20MB hard-drive, 640K RAM, one floppy
drive, XT but with a built-in CD-ROM drive, using DOS 3.3 of course, with
maybe 4.0 as a possible upgrade option.  That company was later bought by
Philips in an ill-fated attempt to enter the home computer market possibly
using its Magnavox brand name.  I believe the CD-ROM component of that
machine was value at something between about $800 aod $1,000 at the time.
Of course, they're throwing away 8-bit SCSI cards by the ton now, so the
SCSI option might be believeable.  Of course, you don't even want to think
about it on an XT with any less than 640K of memory.

Why would you want to do such a thing anyway?  Probably for the same reason
that is often given for why a male dog licks himself between his hind legs;
"because he can."
