Two mail transport agent I know to overcome the problem are Colston's
"Netmail Pro" - which, however, is integrated with its own packet
driver - and Marc Ressl's Netmail:

This latter one does allow indeed to switch between different SMTP
servers, which each use their own (anti-spam) authentification, on one
and a same dial-up to the net access provider.
The setup (with dosppp, and the in-/out-box directories) and mail file
formats seem quite the same with smtp so it could be easy to exchange.
The author, Marc Ressl is actively working on it with including
further functionalities, like IMAP-ability etc., so it seems that this
is a DOS MTA which will be working for quite some time ahead too.
(Provided there'll be a DOS PACKET DRIVER TOO that keeps up with
present and future condition ... do you listen David ?! - our hopes
rest on you ;).
