- fix the macros for dealing with IVs
+ protect them with braces
+ make the deal with the length variable
+ convert loose memcpy calls into macro calls
+ eliminate a few redundant calls
- eliminated unused st_old_iv from struct state
and add better comments for the other iv fields
- add a bit of explanation to the countinuation structures
- eliminate .pcrc_free field from struct pluto_crypto_req_cont
- prcr_init: ditched non-standard parens around braces around body
- extend prcr_init to set the .pcrc_func field (better discipline, shorter
code)
- got rid of ike_continuations.h. Its contents belong in pluto_crypt.h
