Tidy pluto_crypt
- move enum pluto_crypto_requests from pluto_constants.h
to pluto_crypt.h
- eliminate unused elements of enum pluto_crypto_requests
- don't specify numerical values for members of enum pluto_crypto_requests
- don't specify numerical values for members of enum crypto_importance
- get rid of switch by specializing pcr_init into
pcr_nonce_init and pcr_dh_init
