pluto: improve modularity of hashing functions (as suggested by Andrew)
- eliminate HMAC_*_BLOCKSIZE macros by manual substitution.
These really aren't abstractions.
- document lack of documentation for the correct bound for ckaid space
- add passerts to check that MAX_HMAC_BLOCKSIZE is sufficient.
New hash functions might make this manually configured value wrong.
