msgids: move #defines to defs.h, add PRI_MSGID and rename
v2_INITIAL_MSGID->v2_FIRST_MSGID
Group the msgid_t defines with the type definition.
Hopefully PRI_MSGID is easier to remember than PRI?32.
For IKEv2, msgids are ordered, and "first" should help convey that
knowledge (yea, first is actually zero :-)
