I wish ASCII had included both a left and a right quotation mark... but I suppose it is too late for that.

use of OSIS <q> in lieu of actual quote marks is a bad practice. Characters, including quotation marks, are data and should be represented as character data, not markup, IMO.
We can support translation of <q> into some kind of quote mark if we want to, but I can assure you that I, personally, will never create a module that does not represent quotation marks as quotation marks.
So your modules would have literal left and right double quote marks or left and right guillemets, exactly matching the original source text ? That seems like a reasonable thing to do.
I always thought that the rules my English teacher tried to teach me were silly, when the rules involved adding or changing text when I put it inside a quote.
