I think if we let people create new users while logged in as another user they'd..get confused and create new users all of the time?
I like Brent's suggestion. "Sign your name here for access." That, or maybe adding something to the command-line maintenance utility for user creation? Letting logged-in users create new users is kinda weird, because we'd want to only allow admins to do it -- and then we'd really only want to allow "super-duper"-admins, and we don't have a concept of that right now (global, not just an admin of a single wiki -- in your case it's the same).
