I would agree with this 'proposal', seems to handle all situations even covering 'traffic calming' techniques where a lane is deliberately obstructed and traffic needs to wait for a break in oncoming traffic. The one area it doesn't cover is the merging of lanes I mentioned earlier, as I have seen it describing lane counting from the 'slow' lane. In a non-segregated road I suppose you could count from the slow lane in the direction of the way, but then we're back to the switch directions problems.
