I think we are in danger of having a million top-level tags, and that trying to be a bit more hierarchical is better. What about relation on all the buildings with amenity=hospital, or a polygon on the grounds, and then a point for the main entrance with amenity=hospital and a point for the ER entrance with amenity=hospital emergency=yes? I generally lean towards the simplest adequate scheme.
