I don't think it's a phishing attempt, apart from giving your email address at the end if you want to get an amazon voucher, there is not much that can be used to identify you.
I have to admit, the questions were a little odd and somewhat personal, but guessing the research is what motivates people to contribute and what sort of people they are.
