There's nothing wrong with the template, so it's likely something
else. The most common thing being that you have a duplicate entry
for the same address using the template and the duplicate entry has
no template defined for it.
The other possibility is that the address book entry has the
address listed more than once in its properties. This happened to
me on more than one occasion.
Finally, if you don't have a default template defined, then it's
likely the address book specific ones will not work.
