It's usually a server configuration error.
BTW, you've just committed one of my pet-peeve errors -- starting a new
message by 'replying' to a different thread. For those of us who thread by
references, your message is tagged on to the existing thread rather than
starting one of its own. (Datula has a button to detach a message from its
existing thread; I couldn't find such a feature in The Bat! though.)
Set up your per-folder or per-address templates properly and you wouldn't
have to do things that way.
