Because one of your filters misbehaves. Take it again step by step: 1. You send out a message from the Outbox. 2. This message ends up (automatically) in the Sent Mail folder. 2a. Only if you use a filter for Outgoing mail something else can happen (i.e. moving the message to another folder or getting deleted). To see if a message has reached the receiver you can use the Confirmation (Received/Reading) or ask them personally. None of theses options ensures that a message is not lost somewhere on the way - the same as with normal mail.
