IIRC F-Prot had a commandline interface, so you should be able to
create a filter that extracts the attachment, make F-Prot scan it and
afterwards delete it.
Should be (one of) your first filter(s), make it a common filter, so
you'd need just one. Don't forget to set the 'continue processing with
other filters' option.
