Does it need to be self-extracting (which implies that it's zipped or 
otherwise compressed)?

What I do is create an Index.htm page on the CD and have a link or links 
from it to files in a directory.  It may not run automatically, but when 
you look at the list of CD files, it's the only one to open.

You could create an autorun file that automatically opens the index.htm 
file, but googling would show you how to  do that.
