In meetings, where I value interaction with people over interaction
with suboptimal technology like Palm Pilots (tried), Ipads (tried) or
laptops (tried), I use pen and paper. After the meeting, I transcribe
my remarks into actionable items in todo lists, send messages, etc.

For more structured notes, I use Mediawiki, which allows me to
categorize, attach files, etc., without forcing me into any structure.
We have all kinds of conventions for our wiki, but I have my own
notebook-style pages for  my own notes.
