Personally, I think this is a job for wireshark. Sniff the external interface, and make sure the request is arriving. Then sniff the internal interface of the firewall, and make sure that inbound request is getting NAT'd through the firewall correctly. Then sniff the interface of the server, to see the packet arrive there. And watch the packet go out from the server. And arrive at the internal interface of the firewall, And finally leave the external interface of the firewall.
