Regarding the 2-bar version, shouldn't the symbol really have a bar line
running vertically through it? If so, I'll leave that as an "exercise for
the reader" for now.

I hope this was fixed by updating your musixtex.tex . The macros you were
missing are a result of some work Werner and I did to upgrade voltas, which
led us also to correct some flaws in the space-accounting for various repeat
symbols, which in turn led to some further corrections in \centerbar . It
was a very interesting collaboration because our roles were reversed from
what had become normal: usually Werner would suggest some upgrade for PMX
and maybe give some ideas how to implement it, and then keep prodding me
until I got it just right and covered all the possible variations. This
time it was I who showed Werner how to get started, and then I prodded him
to deal with some details he had just let slide. The net result of the
collaboration was the same as usual in the sense that we accomplished
something together that probably would not have happened otherwise. There
is a comment about these mods near the top of musixtex.tex .
