Rainer-- the pmx source that Andre provided doesn't work with pmx 2.3, but
is OK with 2.357a, the latest beta.  I'll email you the TeX file privately.

Andre-- You're right, when you use two D's on the same note, the text for
the second one gets clobbered by the first.  I'll look into what's required
to fix this.  In the meanwhile, you could either define a second line of
music on the same staff, or post the second text string from the other
staff.
