I must confess I don't remember exactly which notations and situations give
MIDI ties and which don't. So I did a test. With the following file:

only the first bar failed to get a MIDI tie. When I uncommented Ap, all bars
got MIDI ties. Unless you had multiple simultaneous ties in a voice, I guess
that means I cannot reproduce your problem (maybe you should send an extract
of your source that you've confirmed exhibits the problem). If you did have
multiple ties, then there's no surprise; PMX cannot do multiple simultaneous
ties in a single voice.
