It's not terribly well specified, but in essence when you type command foo, the 
system looks for foo.exe, foo.com, foo.bat and then foo.cmd in that order. 
After that it tries all the (other) extensions listed in PATHEXT, in order 
(AFAIK, PATHEXT has to be edited from the System Applet and you have to reboot 
for changes to take effect).
