In principle, yes. Just load a file redefining certain critical macros to emit
MIDI rather than to produce DVI/PDF output.

In practice, no. Nobody has written that file yet and I estimate the work
needed as comparable to rewriting MusiXTeX from scratch.

However, if the MusiXTeX code was produced by PMX, just a few extra
lines will produce MIDI.

But wait. MusiXTex produces printed music. There is software that can scan
printed music to MusicXML. Dieter Gloetzel has written a MusicXML to PMX
translator. It's a long and tortuous journey, but the software at each stage
already exists.
