I know you can display images in tiddlers and there are plugins that allow 
simple drawing/sketching in a tiddler, but is there a way to draw over top 
of an image, and save only that overlay (which I assume would reduce file 
size) so that after several are made you can display them all together side 
by side but not have to actually edit the original image file?

Use case:
Patient comes in and I treat specific muscles over the forearm e.g., and I 
can look back over the previous visits to compare which specific muscles 
were symptomatic over the succession of visits.
