To simply insert an image in a tiddler, see Images in WikiText 
<http://tiddlywiki.com/#Images%20in%20WikiText> for the 
*[img[path/to/image.jpg]]* construct. There are other, more advanced 
methods using ViewTemplates, if that answer doesn't go deep enough for you.

If you want to add an image so that it appears below the tags and above the 
tiddler's text field, for instance, you can edit the list field of the 
*$:/tags/ViewTemplate* shadow tiddler 
<http://tiddlywiki.com/#ShadowTiddlers> to insert your chosen template 
tiddler <http://tiddlywiki.com/#TemplateTiddlers> which contains the 
image(s).
