Here’s a very rough start. Try the following in your stylesheet:

body.tc-body .tc-tiddler-title-icon {
background: <<colour background>>;
position: absolute;
margin-left: -2em;
}
body.tc-body .tc-tiddler-title-icon svg,
body.tc-body .tc-tiddler-title-icon img {
width: 2em;
height: 2em;
}
---
