Custom Fields are added to tiddlers either when they are created, or after 
they are created.

When you create a new tiddler, towards the very end there is an area to: 
Add a new field
Fields created there are custom fields.

However your specific question is to: add a custom field to *TiddlyWiki*
This request is not clear.
