Interesting problem. One way to go might be to use only the noun as the 
title of the tiddler but add the article by means of styling(!). The 
LinkStyle <http://linkstyle.tiddlyspot.com/> plugin would enable this. This 
way it would read e.g "la valentina" but the actual title is only valentina 
including when it is sorted. Using the linkstyle plugin you'd add a field:

linkstyle-before :  content:"la "; font-weight:300;

Some tweaking might be needed.
