I suspect this problem is distributed based on the wealth of students, or at least their access to credit. For a custom prepared paper, the cost is not trivial. Given how many of our students can't afford textbooks, putting similar amounts of money towards an undetectable paper seems most likely to be comparatively rare. Getting writing samples prior to high stakes papers for comparison can combat this a bit.
