I see no case in the linked examples in which the title/author/affiliation are 'flush against the top margin' (that would make it 1" from the top edge of paper) and I don't allow it in my class because it isn't what APA shows. Because the APA is vague, I am flexible for it to be between about 2" to being centered on the entire page, but rarely take off points for it unless the student is all over the place in their APA formatting effort. I don't request author notes (though I had a student write a humorous one once about being from Mars, LOL!).
Since I grade all of my papers electronically, it is irrelevant where the names are. I'm not thumbing through papers or handing them back in the classroom.
