Pricing varises. If you buy a box at retail, or off someone else,
then the montly fee is $12.95 for the first box ant $6.95 for each
additional box on the same account. Or there are 1, 2, and 3 year
service-only pre-paid plans.
If you buy a box from TiVo.Com then you get a bundle, and that is
$19.95, $18.95, or $16.95 a month for 1, 2, or 3 year commitments - or
there are also pre-paid plans for bundles.
But tat's it, there are no additional fees from TiVo.

No, but it is advisable to do so. And if you have a broadband
connection it is better to put the TiVo on the network than to use the
phone.

Approximately once a day, but it'll vary a bit.
