Okayyyy,

This is classic...

I've been working with computers for fifteen years and it happens every
time. It`s when you give up and ask for help that you find the solution
(usually when your colleague looks at the problem trying to help, and
usually it's pretty dumb too).

The problem was in the way I had defined my qmail in the users/assign file.

I had my test account set up like this (don't ask why, I wonder myself, I
was trying things out), two lines:
+mydomain-test-:pop3usr:503:505:/var/qmail/popboxes/mydomain/test:::
=mydomain-test:pop3usr:503:505:/var/qmail/popboxes/mydomain/test:::

For some reason, though the qmail home dir is the same and therefore it
would have used the same .qmail file, the behaviour was different when the
mail was sent to test-confirm-etc.. or sent to test.

When sent to test, tmda would kick in, when sent to test-confirm, it would
not and would simply deliver to the Maildir right away (so the confirmation
was never processed). I wonder if this is a qmail issue or a tmda issue. But
it is now fixed by having only one line like this:

+mydomain-test:pop3usr:503:505:/var/qmail/popboxes/mydomain/test:::

So Jason, thanks for your suggestions, I hope my lost hair will serve
someone who decides to search the archives one day :P.
