I guess you do not have access to the qmail logs? If you do, take a look in
there, they might give an idea of why the delivery failed.

Is the message delivered at all to the user (= without confirmation request)
or does it seem to be lost?

One thing I noticed is tmda requires to chmod the crypt_key file to 0600 or
0400, check that too.

And finally, check for a TMDA_SOMETHING in your home directory it may
contain error information.
