FWIW, I'll give you my personal preferences on the above.  These
choices are really unrelated to TMDA, as TMDA should work equally well
with any combination you choose, with the exception of a
Sendmail-based system.

MTA:

qmail seems to have the best "virtual user" capabilities with packages
like vmailmgr and vpopmail.  If this is very important to you, you
might want to stick with qmail.

Otherwise, Postfix is my current MTA of choice.  I switched to Postfix
last September after using qmail for over 6 years, and have been very
happy with it.  Postfix serves all my needs yet is still "simple"
enough that I feel I have a clear understanding of how it works, and
how to leverage its capabilities.  It is also actively developed by
someone who is generally amenable to suggestions and patches.

Exim looks powerful, but its overwhelming number of features and
complex configuration turned me off.  Exim's security track record is
also not squeaky clean like Postfix and qmail's are.

Courier (based on qmail) looks promising, but its author is difficult
to work with (IMO), and development progress and responsiveness tends
to be a bit slow for my tastes.

Sendmail is an abomination that I wouldn't recommend to my worst
enemy.

POP3 vs. IMAP:

This depends on your needs.  IMAP is a storage protocol, whilst POP3
is a retrieval protocol.  If you don't mind your users storing mail on
the server, IMAP is probably more versatile.

mbox vs. Maildir:

This is an easy one for me since I don't like losing mail.  mbox files
are not safe or reliable, and Maildir is.  Case closed.  For
Maildir-based IMAP servers there is Courier-IMAP, which I've used.
I've also heard good things about Binc IMAP (http://www.bincimap.org/).
