I understand, but how about selectively allowing mail to
user-confirm-* so that TMDA works while still refusing mail to all
other unknown addresses?  Does it have to be all or nothing?

How about asking the author of the patch and/or the qmail mailing list
about this issue first?  It's possible someone else has into it
before.
