Hi Ted I haven't followed in detail this thread but I can really recommend the tool AutoRuns for Windows (from Microsoft sysinternals) http://technet.microsoft.com/en-us/sysinternals/bb963902.aspx
The tab "Explorer" also lists ShellIconOverlayIdentifiers and related things. This might help diagnose your problem.
