1) It probably downloaded firmware from MySqueezebox.com at first. Then,
when connected to your local LMS, it may have downloaded even newer
firmware from LMS. Not really sure. No need to switch back and forth
between LMS and MySb.com though. Stay at LMS, and the Touch will stay
with the firmware corresponding to your LMS version.
2) I suppose it's not. It's an odd scenario, though.
3) Yes, go to Settings->Advanced->Networking-Remote Libraries. But why?
Doesn't the Touch find your LMS? If it ain't broke, don't fix it.
