Well, good news. I applied the patch according to the recipe
(application_pool_race_condition_fix.txt), and the problem seems to have
gone away!
(same configuration as before, using 1.3.0)
I have not tested this on our server yet, but on my local machine running
10 browser-emulation processes banging away at the local server app (each
making 200 requests), not a single error! (2000 requests in about 10
minutes)
Will try this on our server late tonight.
