The attachment buttons should appears when the user has ATTACHMENT_CREATE. ATTACHMENT_CREATE is granted when the user has WIKI_MODIFY. The only factors I can imagine that would result in the behavior you are seeing: * Permission policy such as AuthzPolicy revoking WIKI_MODIFY for certain paths * The attach file button is hidden by JavaScript. You could inspect the page source for a page where the button is hidden. * You have some site templates that are out-of-date.
