It is called out in the Form 730 application guide, Exhibit G. You
are also required to sign a declaration when submitting for Part 68
that states you will comply with the Continuing Compliance
requirement.

Hello Treggers:
Can someone point me to the section of FCC Rules Part 68 where retests of
registered wequipment is required every six months for Continued compliance?
We have endeavored to do this, but in searching Part 68 I may have missed it,
but have found nothing to that effect. (Other than they must continue to
comply)
