You seem to have had your RJ question well stomped on. Suffice it to say 
that the connector and pinout for the S/T interface is the same in the US 
as in Europe. I believe that the electrical interface is the same as well.

Now to address the protocol question. I believe that you could get your 
product approved in the US and Canada using the CTR 3 protocols, since 
the requirements here for approval are very minimal. However you won't 
find any publicly available lines which use this protocol. The US took a 
different track in setting the intreface standards and you will need to 
research these. I would suggest that you look at the Bellcore, now 
Telecordia or some such, standards NISDN 1 and, maybe NISDN 2 to find out 
what's required. There are probably ANSI standards as well which cover 
this.
