WebOSInternals is currently rewriting how themes are done, but they currently do not have the same technology that allows for patches to be updated after a new OS update. They have stated that you should definitely uninstall themes before an update, but as long as you have an up-to-date version of preware and patches, you don't have to uninstall any patches.
