Where does the album art reside? I see the individual image files on the
Pre, but where are they on the PC?

I don't know the answer to your question, but here's some advice on
keeping it from happening in the future: embed the cover art in each
MP3 file. You can do this easily by getting a free tag/art management
app called MP3Tag (http://www.mp3tag.de/en/). Highlight a bunch of
tracks, drag a cover art image onto the blank spot in the GUI, and
click "Save" -- each MP3 file will now have the cover art embedded
within it, so you won't have to worry about losing associations in the
future. HTH.

the album cover art thumbnails are missing. I later found all the thumbnails
in the Pre's photos app! Hundreds, and many multiple copies.
