I have the 450 minutes, $69.99 Everything Data plan for my Palm Pre right now.  
Here's the breakdown of my bill:

Plan: $69.99
Equipment protection: $7.00

Sprint surcharges breakdown below:
1) Federal-Univ Serv Assess Non-LD: $1.12
2) Wisconsin State-Tele Sales Surcharge: $3.10
3) Wisconsin State-Univ Serv Assessment: $0.57
4) Milwaukee County-Tele Sales Surcharge: $0.37
5) Administrative Charge        $0.99
6) Regulatory Charge    $0.40 
Total Sprint surcharges (1-6): $6.55

Government Fees & Taxes: $0.75

Total Bill: $84.29 for the "average Joe."

Through my employer, I get a 25% discount off the main plan.
$69.99 - $17.50 = $52.49
Which makes my total monthly bill = $66.79
