I asked a Netflix representative about this very question today. I was told
that when the current gift card expires (actually a little before) you will
get an email asking if you want to apply another gift card. If you respond
yes, you will then be able to go to your account online and apply the gift
card to your existing account, at the existing rate. You will not be able to
change plans, but you can keep the one (presumably combined disc/streaming)
you are already paying for with a gift card, and in fact can continue as
long as you have cards (though it is not clear how long you can continue
extending this - I guess they are not selling any new cards of that
denomination, though maybe if you bought up a whole bunch on the aftermarket
you could keep your current combined plan/rate for a long time?).
