He never had a medical license - he had a psychology license (he is a clinical psychologist). He gave up his license some years ago because he was not always playing within the lines of the American Psychological Association guidelines, and his entertainment work put him at risk for violating the terms of his license.
Technically, being a "doctor" is a function of education and degree earned (medical or academic), whether or not one has a license.
