That is interesting- I wonder if the difference is due to discipline or
country? In the US, a person who earns their JD is a lawyer, whether or not
they have passed the bar and are able to practice law. SImilarly, someone
who earns their MD is a doctor, even before they pass the licensing exam
(similarly with Psychology).

This does create the kind of confusion we have had here - McGraw can
accurately present himself as "Doctor", leading his TV audience to think he
is a licensed professional (the small print scrolling quickly after this
show explains (does he still have a show on the air?) that he is not
licensed, and is only an entertainer, but of course few notice this.

I should note an informal but important caveat: In many (most?) teaching
hospitals, the term "Doctor" is used by and around patients to refer to the
role and function, not the degree. So a 4th year medical student doing a
clinical rotation, who has not yet earned their degree, is usually referred
to as "Doctor" by both patients and nurses (at least when the latter are in
the presence of patients). This no doubt it in the service of the ongoing
mystification of patients, so that they ascribe god-like powers to anyone
treating them.
