You know part of the problem IMHO is that it's a fine line to walk between promoting 
from within, and seeking outside expertise.  A company that only promotes from within, 
ends up with people in positions for which they are not suited.  A company that only 
seeks outside expertise, ends up with experts who don't really understand the 
company's business or culture.

You really need a mix.
