We have experienced the same error with Unidata on Windows. The system has a local user for each domain user, who will access the system, but the passwords are different. When the user name is entered without the domain name prefixed, Unidata first tries the local user. The password is incorrect, so the domain user is then tried. So for each correct domain login there is a failed local login.
Within a certain amount of time a certain number of incorrect local logins will cause the account to be "disabled" for a timeout period. HTH.
